Invcestigtaion of clinical applications of APACHEII scoring system for stroke patient
OBJECTIVES: To evaluate the severity of the stroke patients using the Acute Physiology and Chronic Health Evaluation (APACHE) Il scoring system. BACKGROUMD: APACHE11 has been shown to be an accurate seasure of severity of illness ard correlates strongly with patient outcome. However there have been no published reports of its val idation in stroke patients. ETHODS: The APACHE 11 soores of 422 stroke pat i ents in our hosp ita l dur ing 1991 ～1997 were computed on days of 1 ～2 and 3～6 after the admi ss i on. RESULTS:422 stroke patients were included in the retrospective clinical study of APACH4E 11. The higher the APACHE11 scores, the severer the disease, the poorer the prognosis. For prediction of patient”s hospital death, the best cut-off points were 17 for APACHE 11 on days of 1～2 and 3～6 after the admission The area under receiver operator characteristic ( ROC ) curve were 0.815 and 0.868, respectively. DISCUSSlON AND CONCLUSIONS: The APACHE 11 scoring system provided an good means to assess the severity of the stoke and predict the prognosis of the stoke. was a sansitive and reliable predictor of the prognosis of the stoke. Consistent observation of APACHE 11 scores can provide a useful mean of monitoring the course of the stoke.
